2025 Forms 1095-B and 1095-C (Affordable Care Act)
The Form 1095-C (Affordable Care Act) for Tax Year 2025 is now available online through HR/CMS Employee Self Service Time and Attendance. Forms will be mailed by March 2 to employees who did not elect to receive their forms electronically.

To review or print your Form 1095-C in HR/CMS Employee Self Service:
- Log in to HR/CMS
- Navigate to: Main Menu > Benefit Details > View Form 1095-C
- Click on the tax form (highlighted in blue). Filing instructions are included with the form.
Please note that Form 1095-B (Offer of Health Insurance Coverage) will no longer be automatically mailed by the Group Insurance Commission (GIC). For questions regarding Form 1095-B, please refer to the Form 1095-B FAQs.
Important: Forms 1095-B and 1095-C should be retained with your tax records. Do not submit these forms to the IRS.
To access and print Form 1095-C online, you will need a device with internet access and a compatible web browser (such as Microsoft Internet Explorer 11 or higher, Firefox 52 or higher, or Safari 10 or higher). You will also need software capable of opening PDF files, such as Adobe Acrobat Reader, Word, or WordPad.
