NOW AVAILABLE: Submission of Receivable Modification Request Form through DocuSign

Effective Monday, September 19, the Receivable Modification Request Form is accepted by the Office of the Comptroller (CTR) exclusively through DocuSign.

Departments must submit a Receivable Modification Request Form to request a decrease of the dollar amount on an existing line on an original RE document by $100,000 or more.

CTR has employed DocuSign for departments to complete and submit forms more efficiently, quickly, and securely.

The new DocuSign PowerForm for the Receivable Modification Request form is available on the CTR Intranet Electronic Signatures page [MAGNet or VPN requred].
