Effective Monday, October 17, 2022, the Submission of the Procurement Card Employee Use Agreement and the Procurement Card Setup Form will be accepted exclusively through DocuSign.

The Procurement Card Employee Use Agreement is agreement outlining the employee’s responsibilities when accepting a P-Card.

The Employee Procurement Card Set Up Form is completed to request a P-Card.

The Office of the Comptroller has employed DocuSign for departments to complete and submit forms more efficiently, quickly, and securely.

Departments can access both forms on the CTR Intranet Electronic Signatures page [MAGNet or VPN required].
